Effective case-notes, file-notes, client reports and operational documents are fundamental tools for workers and organisations to keep track of and communicate about what’s happening with clients and programs. They are part of the basis of decision-making about clients. Most importantly, if anything goes wrong, they may be called as evidence in reviews or legal proceedings.
